It's my understanding that skill mod effects are all limited to +25 max. So even if you had two +40 tapes for melee defense and put one on pants and one on a shirt, which looks like +80 you'd really only get +25. I'm not sure if the Professions window in the UI with skills list is reflecting the +25 cap or not, I have only tested it with Def V. Stun (which correctly caps on the UI at +25).
For dodge/counter/block/acuity: 2 things: I believe Natural skill in these categories on live were capped at 125 (anything over this is non-effective) but can be modded by another +25 by using tapes for an effective total of 150. At some point I believe they changed it so it was hard-capped at 125 regardless of tapes, but I'm not 100% sure of when.
It is my understanding that avoidance skills are limited to having equipped weapon type they come with. So Dodge=Pistols/Fencer only, Counter=Carbs/Swords, Block=Pike/Rifle and Acuity=TKA
If I recall, that Melee/Ranged Defense and Defense V. States are the defenses that don't account for the weapon you're using.
LS Should have it's own skilltapes unless I'm mistaken, If I am please someone correct me.
